Developing a Program Framework and Performance Monitoring Plan
Health care systems are increasingly developing medical management or care coordination programs, implementing new technologies, and providing additional support services to address barriers to care faced by vulnerable populations. This Off the Shelf TA project will help organizations better articulate and monitor their initiatives. It will also allow staff involved in implementing these programs to build upon successes and identify areas for improvement. ICH will offer TA to develop a detailed visualization of the programmatic framework through two strategic facilitated conversations with key leaders and/or staff. After finalizing the framework, ICH will work with the ACO/CP to create a detailed work plan with implementation milestones and a performance monitoring plan. Up to two revisions of the work plan and performance monitoring plan will be included.
Depending on the complexity of the initiative and the number of people involved in its implementation, ICH is able to increase the number of facilitated discussions about the program framework, and the level of TA support and revisions in developing the work plan and performance monitoring plan, as needed.
