Evaluation is a crucial component of Quality Improvement (QI). Establishing methods for evaluation can help your team identify barriers to care, develop and optimize interventions, re-allocate resources and reduce quality gaps to improve project outcomes and success rates.

Medicalincs QI experts will support ACOs and CPs to assess the effectiveness of population health management QI activities, understand how data is used to inform QI, understand process evaluation, develop indicators for evaluation, and identify how to tailor changes to improve your organization’s quality improvement interventions.

For sustainability, this project will also include targeted training on:

• Selecting evaluation questions and indicators
• Prioritizing indicators that accurately reflect process and outcomes
• Developing reports and a timeline to share interim results
• Developing tools and resources to accompany results
