Transitional Care Management (Episodic Care Management)
Episodic (short-term) care management is provided when patients are undergoing transitions in care. Episodic Care Management interventions can be a critical value add to beneficiaries with managing care coordination, reducing emergency room visits, and inpatient readmissions; and reduce avoidable utilization costs. The Medicalincs Team can work with your team to identify strategies and interventions to establish sustainable episodic care management interventions. This Medicalincs Off the Shelf project, offers you a tailored approach to meet your needs. Some of the deliverables you can expect on this project include:
- Sustainable intervention(s) to address post-discharge follow-up to beneficiaries recently discharged from the emergency room or inpatient hospitalization.
- Workflow integration of episodic care management with existing care management processes.
- Performance Dashboard for ongoing evaluation of transitions of care efforts.
ACOs and CPs will have the opportunity to customize the episodic care management workflows developed by Medicalincs to align with their current processes. Medicalincs does this by using its EAID model approach that assesses current processes and policies before developing a sustainable solution.
