Care Planning Learning Collaborative Kickoff Webinar

Description: This webinar provides a high-level overview of the purpose and goal of the MassHealth Care Planning Learning Collaborative (LC). The CHCS/Camden team reviews learning collaborative activities and shares findings from key informant interviews that helped inform the design of the learning collaborative as well as providing an overview of how the Camden Coalition will be drawing on their work to support shared cared planning efforts.
