The second SWI Pop Up event occurred on Friday, December 6, 2019 and included representatives from the Stanford School of Medicine (CA), Whitman-Walker Health (DC), and Community Health Innovations of Rhode Island (RI). These leaders shared innovative strategies for keeping individuals engaged in systems of care by lifting up what matters most to them. The discussion highlighted ways for ensuring that the healthcare settings with which people interact are intentionally designed to address their unique circumstances. Participants left the session with key insights to implement or explore further in the context of their own programs.
