This TA project for LTSS Care Partners developed strategies, recommendations and an implementation plan for enhancing collaboration with ACOs. As part of this process, Health Management Associates developed a report on findings from internal and key stakeholder interviews, with recommendations for enhancing LTSS/ACO collaboration.
