Our team led the 5-year Medicaid demonstration waiver in Western NH. We planned and implemented all aspects of a widespread integration program among primary care, acute care, behavioral health, community support providers, schools, and jails including: Clinical Process Redesign; Workforce Capacity Improvement and Training; Technology Deployment; Patient Privacy; Clinical Quality Reporting; Payment Reform; and Multi-Stakeholder Governance.
