Setting ACO Performance Targets with a Health Equity Lens
Faulkner Consulting Group will help you develop performance benchmarks and targets for a set of metrics related to MassHealth ACO implementation. We will leverage our turnkey modeling tool to help you to evaluate disparities in performance versus national and regional benchmarks. Our tools also allow for setting targets by specific demographics – defined by race/ethnicity, language, or other criteria – so that you can differentiate performance goals by population and develop actionable plans to reduce disparities in your ACO or CP performance. The final deliverable will include a set of overall targets and subpopulation targets, plus a user friendly Excel model that can be easily updated and adapted as new data becomes available.
Customization is available with respect to number of TA hours an organization requests to support the project. Total requested hours will influence pricing.
