The TA SOW and Budget Approval Form document is provided below in both MS Word (docx) and PDF formats. TA Vendors will submit a TA project SOW and budget to Abt and MassHealth for review and approval and should also submit this approval form signed by both the ACO/CP and the TA Vendor. The form acknowledges that the ACO/CP and the TA Vendor are in agreement about the TA project SOW and budget.
