Community Complex Care Blueprint Implementation
The Community Complex Care Blueprint supports implementation of enhanced case management services for individuals with complex health and social needs, such as those who are high medical and social services system utilizers, homeless, or precariously housed clients. This Off the Shelf TA offering combines a collection of web-based case management tools, tailored assessments, reporting and analytics tools, process workflow and task management content, and resources for self-guided implementation. ACOs and CPs can leverage the Community Complex Care Blueprint to assist clients with accessing and navigating needed services, initiate closed-loop referrals with local partners, and track and manage cases to completion.
