Housing Resource Inventory
These are customized housing resource inventories of affordable housing and related supports in ACO/CP service region(s) providing availability, eligibility, and access information to help ACO/CP staff, care managers, and care coordinators better understand these resources and respond to the needs of members experiencing homelessness or housing instability. Inventories are customizable based on a basic assessment of staff’s current knowledge base and region-specific housing resource needs and gaps. Optional training/coaching on effective housing navigation is available for an added cost.
