Multi-Visit Patient Method Implementation Coaching Program

The Multi-Visit Patient (MVP) Method is an evidence-based, person-centered, locally adaptable, and operationally feasible methodology for improving care and slowing the cycle of high utilization for MVPs. The MVP Method builds on the lessons learned from large-scale readmission reduction initiatives – taking the best of what works, leaving behind seemingly good ideas that didn’t work, and focusing on identifying the core principles and successful approaches from hundreds of teams – to create a broadly applicable methodology. The MVP Method has been implemented by over 200 sites, allowing for extensive field testing, adaptations, and improvements to how the method is taught. The objectives of this TA engagement are to implement the MVP Method by forming an inter-departmental, cross setting action team, implement the elements of the MVP Method to create a locally-relevant MVP care pathway, and gain the skills needed to continuously improve the MVP care pathway over time.

CHS is happy to customize this TA opportunity in any of a variety of ways. This could certainly lend itself to ongoing performance improvement coaching. We are always eager to teach the MVP Method at the largest scale possible, and would be delighted to customize pricing and format to accommodate multiple ACO/CP teams.
