SDOH Leadership Workshop: Ground and Align for SDOH
Spring Street offers a workshop specifically designed to align leadership around a common vision and goals for addressing social risk factors. We ground the conversation with a custom social determinants of health (SDOH) Data Pack that provides definitions, reference data, and examples from around the industry. Many organizations have been addressing social needs as part of their mission for some time but have yet to expand their work to the scale and rigor needed to significantly impact population health. This workshop helps leaders to align leaders and stakeholders with a common understanding of SDOH and a framework and methodology for adopting a sustainable approach to addressing SDOH. The workshop includes addressing:
- Definitions and goals
- Organizational structure
- Member needs and identification
- Programs and services
- Measurement and outcomes
- Custom SDOH Data Pack
- Workshop Sessions
- Workshop Findings
Spring Street Exchange has the only database in the country that marks the industry’s footprint and progression toward addressing social needs and grounds the workshop in this proprietary data. This provides unique insight on the staffing, organizational structure, data, goals, and other activities that help drive toward a broader, more humane way of supporting Medicaid members.
