Behavioral Health/Substance Use Disorder Assessment and Planning to Reduce and/or Eliminate Unnecessary Hospital Admissions and Readmissions
This TA project for Wellforce in partnership with Fallon Community Health Plan helped them identify organizational facilitators and hurdles to reduce and/or eliminate unnecessary hospital admissions and readmissions. Advocates for Human Potential conducted key informant interviews and developed findings, best practices, and an action plan to address behavioral health care gaps in the ACO’s system.
